Job description: Core Prescribing Solutions is looking for experienced, dynamic and self-driven Clinical Pharmacists who are well-versed in the roles and responsibilities of a pharmacist, including clinical medication reviews, chronic disease management, and providing pharmaceutical expertise, to join our ever-growing organisation.
Role Requirements (Clinical Pharmacist):
Applicants MUST have previous general practice experience.
- Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
- Knowledge and awareness of legal, ethical, professional and organisational policies/procedures relevant to the role
- Qualified or enrolled on CPPE Clinical Pharmacist Training pathway (although this will be offered for PCN roles)
- Experience in supporting common acute and long-term conditions
- Knowledge, awareness and experience of brief intervention and referral pathways that provide opportunistic advice and support for preventable illness and/or long-term conditions receptive to lifestyle/pharmacological support
- Strong clinical leadership, communication and interpersonal skills
- Ability to apply effective clinical judgement in the prioritisation and decision-making aspects of the role
Role Responsibilities (Clinical Pharmacist):
- Contribute to the improvement of health outcomes in long-term conditions as a member of the multidisciplinary primary care team
- Provide specialist knowledge and advice on pharmaceutical matters, including cost-effective prescribing and clinical guidelines for disease treatment, and to provide evidence-based education and training for colleagues in partnership with the practice
- Review patients’ acute medicines requests
- Complete discharge and clinic letters

Job description: Core Prescribing Solutions is looking for experienced and highly motivated pharmacy technicians to join our expanding team where you will have access to peer support and development opportunities as part of our MDT.
You will be a great team player, but capable of independent and autonomous work. You will work as part of the multi-disciplinary team to support member practices with medicines optimisation, clearly understanding and adhering to defined pharmacy roles and responsibilities.
Role Requirements (Pharmacy Technician):
- Support the PCN clinical pharmacists in the delivery of clinical services including processing discharge/clinic letters from secondary care
- Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the clinical pharmacist/s
- Assist with medication safety monitoring systems (e.g. high-risk drugs)
- Liaise with clinicians and administrative staff to resolve medicine-related queries
